CarePointe Care Management provide you with a customized experience based on specific needs. Our care coordinators build relationships with each of our patients—to keep them motivated and ensure continuity of care. Chronic care management offers additional help managing conditions like arthritis, asthma, diabetes, hypertension, heart disease, and osteoporosis. Services may include:

  • At least 20 minutes a month of CCM services

  • Personalized assistance from a dedicated health care professional who will work with you to create a care plan

  • Coordination of care between your pharmacy, specialists, testing centers, hospitals, and more 24/7 emergency access to a health care professional

  • Expert assistance with setting and meeting your health goals

Medicare introduced the Chronic Care Management to provide better healthcare, outcomes, reduce unnecessary medical expenses, and improve care coordination and management for patients. Medicare reimburses physicians for providing 20 minutes of non-face-to-face Chronic Care Management every month!

Who Is Eligible?

Chronic care management services at least 20 minutes of clinical staff time directed by a physician or other qualified health care professional, per calendar month.
