Carepointe leads the industry with Remote Care Management

Founded in 2015, Carepointe has been the industry leader of technology-enabled care coordination. Their Value-Based Services help facilitate the set up and ongoing collection of patient data through remote monitoring.

Additionally, their Chronic Care Management (CCM) is a Medicare service for beneficiaries with two or more chronic conditions such as diabetes and hypertension.

When used in conjunction with each other, Carepointe’s Remote Care Management can provide an extra level of care for patients who have chronic conditions and need help tracking vital signs outside the office visit to stay compliant with their care plans.

For those unfamiliar, Remote Care Management should not be confused with telehealth. Remote Care Management is so much more as it combines the elements of Chronic Care Management with Remote Patient Monitoring, enabling physician practices to track, record, and review patient physiologic parameters such as weight, blood pressure, pulse oximetry, and respiratory flow while a patient is at home. Remote monitoring can be an effective method in identifying patient issues for early intervention.
